Recommendations

To request a recommendation:

1. Download the recommendation form and fill it out completely.
2. Visit the ODASIS staff three weeks prior to the due date of the recommendation.
3. Please bring a copy of your transcript with your name on it as well as a brief, personal description (e.g., resume).
